Reset Alert – Is this for real?

I just received a message in my hotmail/outlook inbox that states "Reset Alert Confirmation." It stats that "we have recently confirmed that different computers have logging onto your Hotmail account and multiple password errors have been entered.  It goes on to ask me for my name, user name, password, date of birth, etc., and said it is from the Windows Live Hotmail Team.  Is this for real?? Do I need to close my email account?
