I recently became homeless, on the 15th of July, and I had to access my account from a public computer in the interim. The first attempt showed the error in the subject. The two call or text options are not directed at my current phone number. The "send a code via email" is connected to an email I likely created for a halo account way back in the day, and it is blocked when I try to sign in. 

I have filled out the forms for recovery including as much information as possible. I haven't sent many emails recently, but those I do are usually sent to my girlfriend a******** Email address is removed for privacy *** or my father john@**********.com. I have received DOZENS of emails from my girlfriend in the past few months usually with attachments. I have also received a few facebook emails noting messages from her there.  I am on the mailing list of Banana Republic (retail clothing store). I am also on the mailing list of event teams for clubs and parties in my city. In the last few days of June, I sent an email to my dad regarding golf clubs, and an email to my older sister offering my computer to her as I knew I was going to be in a shelter come July. My e-banking reminders are also generally available in my junkmail. Recently they were hounding me to fill out a survey. I think I also get monthly updates from ticketmaster.com for the local upcoming events that month. 
I have a folder called "Folder", which I primarily use to save pictures, and thats the only additional folder I use. 

This is my primary email, and has resume's, bank info, employment info etc scattered in there. 

I have attempted to use your form for recovery twice, but only had the one response to the new email I had to setup. The code for the failed attempt is:  94501893

Please help, as I said, I'm in a homeless shelter, trying to find employment, and trying to avoid losing sanity.
