I accidentally entered incorrect password too many times so the account was blocked.  I was unable to answer the security question either (although I'm 99% certain that it was similar to what I entered - it's kind of hard to not remember the name of your high school...).  Then I had to fill out the "identity verification" form and despite numerous attempts I wasn't successful and finally got a "final e-mail" stating that there's been too many unsuccessful attempts and I wouldn't be able to regain access to my mailbox.  The e-mail was set up 10 years ago so I don't remember the setup details.
The problem with the verification form is that it asks for recently sent messages.  I don't send messages from that mailbox (or very rarely) so I can't provide recent subjects/contacts, etc., however I receive notification messages frequently and would be happy to provide where they come from.  Also - I'm happy to provide the details related to contacts found in that mailbox or some other way to prove that it's my mailbox.  Could also provide a number of possible 4-5 password combinations (one of them, I'm 99% sure is accurate).
